If the advertised role is based in London on an Agenda for Change Pay Band, it will attract a High Cost Area supplement. This supplement is not applicable to Executive or Senior Manager or Medical Leader positions. For positions offered on a part time basis, salary and any allowances will be adjusted pro rata.

Transforming Care – Learning Disability Case Managers – Band 8a

NHS England West Midlands Hub is absolutely committed to delivering the aspirations of the Transforming Care programme.

We currently have an opportunity on a Fixed term contract for a Case Manager specifically for Transforming Care within the West Midlands until 30/09/2019.

We need people who can be professional and challenging, methodical and creative, but above all else people who hold the values and principles that underpin choice and control and who can demonstrate a track record in supporting people to greater independence.

The successful candidate will work within the Specialised Commissioning team based at St. Chad’s Court, Birmingham and support the substantive Case Managers in delivering a high quality service especially with transforming care.

The successful candidate will be responsible for Chairing and having oversight of Care and Treatment Reviews, writing regular updates and representing the team on Transforming Care, as well as case managing the Transforming Care Case load.

The role also requires working closely and collaboratively with other commissioning team members in advancing the collaborative commissioning agenda of NHS England.

You will be accountable and responsible to a Senior Case Manager who with the Specialised Commissioning Head of Mental Health will be supporting you with your day-to-day responsibilities.
