An exciting secondment or fixed term opportunity until the end of March 2019 has become available to work within NHS England’s Hospital to Home Delivery Programme within the Urgent and Emergency Care (UEC) Delivery Programme.

The Realising Every Asset of Community Healthcare (REACH) programme within the Hospital to Home programme works with partners to ensure that community health services are as safe and efficient as possible, focussing on strengthening the relationships with key stakeholders in community health to improve patient outcomes. The programme aims to, across 4 – 8 community areas, provide intensive support to:

  • produce core principles/ specifications for selected community health services including proposed outcomes;
  • achieve 2 day delivery of intermediate care from referral to receipt of care and 2 hour crisis response;
  • identify opportunities to reduce avoidable hospital attendances rates for urgent care sensitive conditions;
  • explore opportunities to provide clinical input and support for care providers managing deteriorating residents;
  • Implementation of a falls prevention train the trainer programme for social care workforce and families / informal carers.

We are now seeking 2 new Project Support Officers to take on the role of supporting the REACH programme. 

The REACH delivery team is the programme support to lead and deliver the REACH programme. The team will link in with the Planning Delivery Team who is responsible for the leading, supporting and delivering the Hospital to Home programme.

The post holder will be someone who has experience of supporting programmes or projects, and someone who is self-motivated, enthusiastic, has attention to detail, is results-driven, has excellent inter-personal skills and the ability to work effectively and collaborative across a wide-range of internal and external stakeholders.
