If the advertised role is based in London on an Agenda for Change Pay Band, it will attract a High Cost Area supplement. This supplement is not applicable to Executive or Senior Manager or Medical Leader positions. For positions offered on a part time basis, salary and any allowances will be adjusted pro rata.

Transforming Care Discharge Co-Ordinator (Case Manager)
Fixed Term / Secondment – 30 September 2019
The East of England Specialised Commissioning Team is seeking to recruit a Transforming Care Discharge Co-ordinator (Case Manager) with significant experience in children and adolescent mental health and/or learning disabilities to support the national Transforming Care programme.
This position will offer the successful candidate the opportunity to work with national and regional colleagues, and providers of T4 services and local stakeholders, with the objective of supporting the delivery of the Transforming Care agenda for those young people with complex needs of learning disability, ASD and mental health.
The Discharge Co-ordinator will work as part of a dynamic team in overseeing the facilitation of care and support packages; and monitoring treatment and discharge plans, to ensure timely, safe and effective discharge from NHS and Independent Sector T4 settings.
You must have appropriate UK professional registration.
