Mental Health Case Manager - Fixed Term Post up until 31 March 2019

North Region Specialised Commissioning Team - North West Hub

Band 8a, Full Time - 37.5 Hours per Week

Applications are sought from suitably experienced/qualified Mental Health/Learning Disability practitioners for a Mental Health Case Manager post. The post will be based in The NHS England North West Hub (Specialised Mental Health Team) and will involve work mainly across the North West area but could involve occasional travel around the country. The post holder will work to support the Transforming Care Programme within the hub and will work closely with the Head of Mental Health and Mental Health Case Manager leading on the programme in addition to the wider Mental Health team to ensure that programme targets and trajectories are achieved. One of the primary functions will be to chair Care and Treatment Reviews across providers of secure and specialised mental health services for adults and CAMHS. The ability to manage multi-professional/multi-agency groups at a senior level to work in the best interests of services users will be a key feature of this role, in addition to working to strict timeframes to produce reports and input data.

 The role includes but is not limited to;

  • Improving quality and outcomes for service users within specialised mental health services through the commissioning process
  • Enabling clinical leadership along the specialised mental health pathway through effective working with clinical teams and case managers
  • Enabling service user involvement in the commissioning process and directly within specialised mental health services
  • Using the commissioning process to promote equality and reduce inequality
  • Working in partnership with other agencies to ensure that commissioned services integrate with the wider health and social care economies
