If the advertised role is based in London on an Agenda for Change Pay Band, it will attract a High Cost Area supplement. This supplement is not applicable to Executive or Senior Manager or Medical Leader positions. For positions offered on a part time basis, salary and any allowances will be adjusted pro rata.

Transforming Care Children and Young People’s Regional Strategic Case Manager South West.
Fixed term or secondment until to 31st March 2019

This post has been created to support the changes in the South Region, and will join the Children and Young People’s regional strategic case manager who works in the South West.
A motivated individual who is committed to ensuring Transforming Care becomes a reality for children and young people is being sought to work in the South West as part of the South West regional team.
Working across the south west region, and linking with the national team, the post holder will be required to work with Transforming Care Partnerships across the South West and to assure their progress and development in implementing “Building the Right Support” and “Developing Support and Services for children and young people with learning disabilities, autism or both.”
The role will involve working as part of a dynamic national team in delivering the outcomes of NHS England Transforming Care Programme
The role is offered as a secondment or a fixed term contract until the 31st March 2019.
