If the advertised role is based in London on an Agenda for Change Pay Band, it will attract a High Cost Area supplement. This supplement is not applicable to Executive or Senior Manager or Medical Leader positions. For positions offered on a part time basis, salary and any allowances will be adjusted pro rata.

Transforming Care CAMHS Mental Health Case Manager
We are seeking an enthusiastic mental health professional with experience in CAMHS services, who wants be part of a great case management team working in NHS England, London region. A fixed term or secondment opportunity has arisen within the Specialised Mental Health Team to accelerate the delivery of the Transforming Care Agenda individuals with a learning disability /Autism.
The Transforming Care, Specialised Commissioning team works to improve health and care services so that more people with learning disabilities and/or autism can live in the community, with the right support, and close to home.
You will be working with our Mental Health Trust Partners to ensure safe and effective care pathways are in place for all young people in CAMHS specialised mental health care settings within the NHS and Independent sectors.
You will provide clinical support and guidance to the Transforming Care project leads in relation to the defined population. This will include project mapping of future service use, identification of users for future service provision and planning in terms if future service needs.
You will also have an important role in working with our quality team to provide oversight into the delivery of transforming care specialist mental health services in London.
The role supports the business in driving transformation as well as value for money in planning, commissioning and service. The role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery.
You must have appropriate UK professional registration.
